Here’s some more info about the activity:

Aim	To practise adverbs of frequency and reinforce the present simple tense.
Activity	Students ask and answer questions and complete a chart.
Organisation	Pair work
Preparation	Make enough copies of chart A for half the students in your class and enough copies of chart B for the other half.
What do I do?	Put students into pairs. Give chart A to one student in each pair and chart B to the other. Students answer the questions for themselves, completing the YOU column. Students ask the same questions to their partner, completing the PARTNER column.
